1 Tee shirt for each swimmer is included inthe registration fee.  Please indicate the size and amount of any additional CST shirts you would like to order.  This year's shirt will be ash with a simple CST graphic.  Ladies tanks are also available, color TBD.  All extra shirts are $10, payable to CST when the shirts come in. extra shirts to order: (# and sizes): ______________________________________________

A copy of swimmers' birth certificate is required if not already on file with CST.  Please have all paperwork including registration form, medical form(s), check to CST, and tee shirt order turned in by May 2 in order to get our tee shirts on time.
